As the form cannot be signed digitally by non-Danish citizens it must be forwarded by post to the Danish Maritime Authority.

This form must be used when a ship with a gross tonnage of 20 or more is to be registered in the Danish International Register og Shipping.
lf the shipowner is not already registered in the Danish Register of Shipping, an application for registration of the shipowner must be submitted.
Proof that the shipowner owns the ship must be sent in, for example in the form of a builder's certificate, title deed or Bill of Sale.
The signature of the yard/seller must be documented. If the ship is acquired from abroad, the signature must be confirmed by a notary public in the country in quest ion.
lf the hull of the ship has been constructed at one yard, and the ship has been completed at another yard, an original builder's certificate from each of the yards must be submitted.
lf the ship is imported from abroad, either an original deletion certificate or an original non-registration certificate from the Register of Shipping of the relevant country must be sent in.
After entering the call sign/distinctive number or letters move on to the next box, we will automatically collect the ship's name and port of registry. If you are unsuccessful you may have mistyped, or the ship may not be entered in the Register of Shipping. In that case, you will have to enter the information yourself.
We hereby declare that the ship will not be used for the transport of passengers between Danish ports. This declaration does not include voyages to and from offshore installations. Reference is made to section 4 of the Act in the Danish International Register of Shipping, which is printed on the reverse.
