Application for certificate conforming insurance or any other guarantee covering civil liability for oil pollution damages from ships
Når ansøgningen indsendes til Søfartsstyrelsen, skal der vedhæftes en erklæring fra forsikringsgiveren eller garanten. Af erklæringen skal det fremgå, at skibet har tegnet forsikring eller stillet anden garanti til dækning af sit ansvar i henhold til 1992 konventionen om det privatretlige ansvar for olieskader fra skibe og inden for den ansvarsgrænse, der er anført i Sølovens § 194. Desuden skal det fremgå, at forsikringsgiveren eller den, der har stillet garanti, bekræfter, at forsikringen eller garantien dækker det nævnte ansvar, samt i tilfælde af garanti, hvori denne består. Erklæringen skal desuden også indeholde ovennævnte oplysninger om skibet. Søfartsstyrelsen kan stille krav om dokumentation for forsikringsvirksomhedens solvens og evne til at opfylde forpligtelser. En elektronisk kopi af forsikringserklæringen (Blue Card) kan accepteres, hvis forsikringsgiveren er en P&I klub, som er medlem af International Group of P&I Clubs.

When the application is forwarded to the Danish Maritime Authority, a declaration from either the insurance company or the guarantor must be enclosed . The declaration must state that the ship has taken out insurance or been provided with a guarantee covering liability as stated under the International Convention on Civil Liability for Oil Pollution Damage, 1992 and within the liability limit stipulated in section 194 of the Danish Merchant Shipping Act. Furthermore, it must be stated that the insurance company or the guarantor confirms, that the insurance or the guarantee covers the said liability and, in the case of a guarantee, the content of this guarantee. The declaration must also contain the above-mentioned information about the ship. The Danish Maritime Authority may require proof that both the insurance company is financially solvent and able to meet the required financial obligations. An electronic copy of the insurance declaration (the "Blue Card") is acceptable, if the insurance company (P&I Club), holds membership of the international Group of P&I Clubs.
