Use this form if you want to establish a tattoing company in Denmark.

You will need this:

Information about the tattooing establishment such as name, address and phone number.
Information about the owner. Either:
- A Danish CVR number of the tattooing establishment or
- A Danish CPR number of you as a owner or
- Other identification documents (passport recommended)
Information about the tattooist working at the tattooing establishment (If the tattooist has a national ID number (CPR), this must be added. In addition, you must enter the name and email address of each individual tattooist)
